Hospital","subitem_description_type":"Other"}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"目的：本研究の目的は、病院外来における在宅療養支援の実践状況と外来看護師の課題認識を明らかにし、課題解決の方向性を検討することである。\n方法：病院外来に勤務する外来看護師を対象として、2018年2月に質問紙調査を行った。調査内容は、①基本属性、②病院外来における在宅療養支援の実践状況、③在宅療養支援体制整備に向けた取り組み状況、④在宅療養支援展開上の課題、⑤在宅療養支援を充実させるために必要な対策、⑥在宅療養支援に対する外来看護師の考え、である。\n結果：359名から有効回答を得た（回収率47.2％）。在宅療養支援全項目の実践状況の平均では、「行っている」および「ときどき行っている」を合わせた平均は39.1％であった。アセスメント項目の実践率が比較的高く、「行っている」および「ときどき行っている」を合わせた平均は、53.8％であった。一方で具体的実践項目の実践率の平均は、37.3％、多職種連携項目の実践率の平均は、28.8％であった。社会資源活用項目の実践率の平均は、24.3％であった。\n　在宅療養支援体制整備に向けた取り組み状況では「既に行っている」と回答した割合は、外来における在宅支援に関する勉強会の開催12.5％、外来で在宅療養支援が出来る組織づくり11.7％等であった。\n　また、92.0％の外来看護師が在宅療養支援は地域包括ケアを推進するために重要と考え、80.0％の外来看護師が在宅療養支援をもっと進めていきたいと考えていた。しかし、81.3％の外来看護師が現在の外来の状況では在宅療養支援を進めていくのは難しいとしていた。病院外来における在宅療養支援展開上の具体的な課題として、看護師の知識やスキルの不足、マンパワーや支援体制の不十分さ、学習機会の少なさ、勤務体制や個人の背景が多様で体制が整いにくいことなどがあげられていた。\n　在宅療養支援を充実させるために必要な対策として、外来の業務整理と人員確保、外来看護師への系統的教育、医師等関連職種の理解、明確な診療報酬の設定、退院調整・在宅療養支援ナースの育成等の項目が高い比率を示した。\n結論：病院外来における在宅療養支援の実践は、アセスメント項目の実践率が高い傾向であった。外来看護師は、病院外来における在宅療養支援は重要と考え、もっと進めていきたいと考えているが、在宅療養支援体制上の課題や看護師の実践力の課題があることを認識していた。そのために、業務整理や人員確保等の「体制」づくりと系統的な教育の展開による「実践力」向上が重要視されていることが明らかとなった。","subitem_description_language":"ja","subitem_description_type":"Abstract"}]},"item_10002_publisher_8":{"attribute_name":"出版者","attribute_value_mlt":[{"subitem_publisher":"京都府立医科大学医学部看護学科","subitem_publisher_language":"ja"}]},"item_10002_source_id_9":{"attribute_name":"ISSN","attribute_value_mlt":[{"subitem_source_identifier":"1348-5962","subitem_source_identifier_type":"ISSN"}]},"item_10002_version_type_20":{"attribute_name":"著者版フラグ","attribute_value_mlt":[{"subitem_version_type":"VoR"}]},"item_creator":{"attribute_name":"著者","attribute_type":"creator","attribute_value_mlt":[{"creatorNames":[{"creatorName":"滝下,
